CPA Covid Waiver

As the home arena for Windsor AAA Zone, Central Park Athletics requires their COVID waiver and screening tool completed daily for entry. Please find the link below, which can be completed before attending for your scheduled ice time. The player and accompanying parent must complete this waiver.
